The name “ComPacks Private describes a community service that provides a suite of care through a professional case manager. It is a service that helps get people home from hospital and provides the home and personal services required to get well sooner.

ComPacks Private is a service providing professional, experienced case managers to work out the support and care that people want and need when going home from a private hospital, or when that person’s health needs and living needs together would improve with the assistance of a case manager.

We start when a person is referred from their specialist or private hospital (and before the person is discharged from hospital) and continues for a short time to set up short term and long term plans.  The case manager works with the person referred (client) and consults any carer or supporter nominated by the client, the client’s GP and your heath care team.

The case manager then rapidly assembles the services the client will need. They will be tailored to the client’s specific needs. The services might include domestic assistance (cleaning, washing, ironing, help with shopping), personal care (bathing, dressing), meals, social support, transport or other services to help you manage at home. The services received by the client will depend on the client’s needs and wishes.

People are eligible to receive ComPacks Private service if they have two or more community care needs.
