ComPacks Private is a case-managed discharge from hospital that includes a package of care for up to 6 weeks after discharge from hospital. This service has been established for people who need two or more community services to ensure they can return home safely with appropriate
care in place.
The concept is to case manage community support jointly with a multidisciplinary hospital team, starting before discharge and continuing for a short time after discharge, with the aim of facilitating access to mainstream community services.

Bronwyn Wilkinson, Director
Bronwyn has broad and varied experience in the Human Services sector in New South Wales and South Australia as both an operational manager and senior manager at Area Health Service and with government services.
Most recently, she was responsible for the management of state-wide programs and contracts totalling $35m including the NSW Chronic Care Program, Hospital in the Home services, the ComPacks service, and a range of strategic projects and initiatives in information technology development, aged care and community care service design and developing integrated approaches between the community sector agencies and acute care services in rural and metropolitan settings.
